Code input before consent form and readme for surveys

Closes #133 (closed)

For the readme I took the liberty of copying and pasting some explanations given by @bridubois in some issues, if that's ok.

I didn't change yet the survey csv as it already changes for issue #138 (closed) I thought it would be easier to change it when that one in closed.
